Timeline of activities regarding PSIC’s file
This timeline shows the activities related to the PSIC file for Case 1, from June 2011 to April 2013
2011
- 27 June 2011—PSIC informs complainant that complaint will be reassessed
- 15 August 2011—PSIC opens file for reprisal complaint; file assigned to investigator
- 23 August 2011–13 September 2011—Investigator and complainant are communicating and finalizing complaint
- 5 October 2011–2 December 2011—No PSIC communication with complainant
- 2 December 2011—Investigator advises complainant that analysis is almost complete
2012
- 5 January 2012–5 June 2012—No PSIC communication with complainant
- 13 February 2012—PSIC realizes complainant’s file is missing and attempts to locate it
- 14 March 2012—Approximate date of recreation of complainant’s file
- 5 June 2012–4 April 2013—No PSIC communication with complainant
- 20 June 2012—Investigator commits to PSIC officials that case analysis will be completed that week
- 23 August 2012—Investigator informs PSIC officials that case analysis is complete
- 25 October 2012—Case management system notes that recreated file is missing
2013
- 11 January 2013—OAG receives disclosure of alleged wrongdoing from complainant
- 15 February 2013—File is recreated for the second time
- 18 February 2013–2 April 2013—Commissioner is actively involved in managing the file
- 14 March 2013—Investigator commits to giving Commissioner the completed case analysis
- 2 April 2013—Investigator provides complainant’s case analysis to the Commissioner
- 4 April 2013—Commissioner writes to complainant to advise that allegations will not be investigated
